JOB DESCRIPTION

TITLE OF POSITION: CERTIFIED NURSE ASSISTANT /PERSONAL CARE ASSISTANT/ CAREGIVER TITLE OF IMMEDIATE SUPERVISOR: RN SUPERVISOR RISK OF EXPOSURE TO BLOODBORNE PATHOGENS: HIGH

POSITION SUMMARY

The Personal Care Assistant provides personal care and supportive services in the client's home under the direct supervision and guidance of the RN Supervisor. All assigned tasks must fall within the caregiver’s scope of practice and be delegated by a Registered Nurse (RN). Under no circumstances may the caregiver perform intravenous, sterile, or invasive procedures, excluding rectal temperatures.

PRIMARY RESPONSIBILITIES

  • Deliver care and services in accordance with the client's individualized Plan of Care.
  • Assist clients with personal hygiene, grooming, and activities of daily living.
  • Support a clean, healthy, and safe home environment.
  • Prepare and serve nutritious meals based on the client's needs and dietary restrictions.
  • Aid in client mobility and ambulation as approved by the care plan.
  • Promote client independence and involvement in daily activities, encouraging physical and mental engagement.
  • Provide emotional support and companionship, fostering trust and confidentiality with the client and family.
  • Accurately document care provided and observations using the electronic system.
  • Monitor and promptly report any changes in the client’s physical, emotional, or environmental condition to the RN Supervisor.
  • Perform light housekeeping and household management tasks related to client care.
  • Collaborate with community agency personnel involved in the client’s care.
  • Attend required in-service trainings and participate in ongoing care planning.
  • Always operate within the scope of practice and follow instructions provided by the RN Supervisor.

JOB CONDITIONS

  • Must have reliable transportation and the ability to travel to client homes, which may not be wheelchair accessible.
  • Requires sufficient hearing, vision, and physical ability to assess client conditions and provide care.
  • Must be able to lift/carry up to 30 pounds and occasionally assist with client weight up to 250 pounds.
  • Ability to bend, stoop, reach, and perform physical tasks related to caregiving.
  • Clear communication skills, both verbal and written, are essential.

EQUIPMENT USED

  • Handwashing supplies, gloves, adjustable bed, Hoyer lift, and mobility aids (walker, cane, wheelchair).
  • Use of password-secured devices such as phones, computers, or tablets to access electronic applications for documentation and communication of client care.

QUALIFICATIONS

  • High school diploma or equivalent preferred.
  • Nurse assistant training with certification by the Maryland Board of Nursing preferred.
  • Must be physically and mentally fit to perform caregiving duties; documentation of health status may be required.
  • Reliable transportation and punctual attendance are essential.
  • Basic knowledge of personal care, meal preparation, and housekeeping.
  • Understanding and respect for client rights, dignity, and confidentiality.
