Electronic Signature and Documentation Agreement
Purpose
This Agreement is intended to inform you about the use and legal validity of electronic signatures and documentation within Care at Home. By agreeing, you acknowledge that electronic signatures on employment forms, patient records, policies, training materials, and other agency-related documents are legally binding and hold the same authority as handwritten signatures under the Electronic Signatures in Global and National Commerce Act (E-Sign Act) of 2000.
Consent
By electronically agreeing to this Agreement and signing any related form or document through our electronic portal or system, you consent to the use of electronic signatures for all employment, clinical, and agency documentation. You agree that your electronic signature, including your Signature Passcode and Login Authentication Password, will serve as your legal signature and authentication for the documents signed. You also agree to protect your login credentials and signature passcodes from unauthorized use and to ensure the integrity and accuracy of all electronic records you sign.
Security Responsibilities
To maintain the security of electronic documentation and signatures, you agree to:
Keep your Login Password and Signature Passcode confidential and not share them with anyone.
Always log out of the system at the end of your work shift or whenever your device is unattended.
Manually enter your Login Password and Signature Passcode each time you access the system, rather than saving them.
Carefully review all documentation online for accuracy before submitting or signing.
Promptly update your credentials if a security breach is suspected or when instructed by the Administrator.
Revocation of Consent
You may revoke your consent to use electronic signatures at any time by submitting a printed or electronic copy of the “Revoke Electronic Signature Consent” form, available in the Forms and Documents section of the portal, or by contacting Care at Home’s HR department.
Paper Copies
You have the right to request and obtain paper copies of this Agreement or any signed electronic document by contacting Care at Home or accessing the Forms and Documents section of the portal.
Technical Requirements
To access and sign electronic forms and documents, you must have:
Reliable internet access.
A compatible web browser (such as Google Chrome or Internet Explorer 8 or higher).
Access to a printer if you wish to print forms for your records.

By signing below, you acknowledge that you have read, understand, and agree to comply with the terms of this Electronic Signature and Documentation Agreement. You accept responsibility for the security of your login credentials and the accuracy of electronic documentation you submit.
